The Man Who Mistook His Wife For A Hat: And Other Clinical Tales, by Oliver Sacks

In his most extraordinary book, “one of the great clinical writers of the twentieth century” (The New York Times) recounts the case histories of patients lost in the bizarre, apparently inescapable world of neurological disorders.

Oliver Sacks’s The Man Who Mistook His Wife for a Hat tells the stories of individuals afflicted with fantastic perceptual and intellectual aberrations: patients who have lost their memories and with them the greater part of their pasts; who are no longer able to recognize people and common objects; who are stricken with violent tics and grimaces or who shout involuntary obscenities; whose limbs have become alien; who have been dismissed as retarded yet are gifted with uncanny artistic or mathematical talents.

If inconceivably strange, these brilliant tales remain, in Dr. Sacks’s splendid and sympathetic telling, deeply human. They are studies of life struggling against incredible adversity, and they enable us to enter the world of the neurologically impaired, to imagine with our hearts what it must be to live and feel as they do. A great healer, Sacks never loses sight of medicine’s ultimate responsibility: “the suffering, afflicted, fighting human subject.”

From Publishers Weekly
A neurologist who claims to be equally interested in disease and people, Sacks (Awakenings, etc.) explores neurological disorders with a novelist's skill and an appreciation of his patients as human beings. These cases, some of which have appeared in literary or medical publications, illustrate the tragedy of losing neurological facultiesmemory, powers of visualization, word-recognitionor the also-devastating fate of those suffering an excess of neurological functions causing such hyper states as chorea, tics, Tourette's syndrome and Parkinsonism. Still other patients experience organically based hallucinations, transports, visions, etc., usually deemed to be psychic in nature. The science of neurology, Sacks charges, stresses the abstract and computerized at the expense of judgment and emotional depthsin his view, the most important human qualities. Therapy for brain-damaged patients (by medication, accommodation, music or art) should, he asserts, be designed to help restore the essentially personal quality of the individual. From Library Journal
Neurologist Sacks, author of Awakenings and A Leg To Stand On , presents a series of clinical tales drawn from fascinating and unusual cases encountered during his years of medical practice. Dividing his text into four parts"losses" of neurological function; "excesses"; "transports" involving reminiscence, altered perception, and imagination; and "the simple," or the world of the retardedSacks introduces the reader to real people who suffer from a variety of neurological syndromes which include symptoms such as amnesia, uncontrolled movements, and musical hallucinations. Sacks recounts their stories in a riveting, compassionate, and thoughtful manner. Written on a somewhat scholarly level, the book is highly recommended for larger collections. New York Magazine Dr. Sacks's most absorbing book.... His tales are so compelling that many of them serve as eerie metaphors not only for the condition of modern medicine but of modern man.

Oliver Sacks as Edgar Allan Poe
By Matt Mayevsky
This is my first book by Oliver Sacks. I must admit I was expecting more science, less "novel." Dr. Sacks writes in a literary style and loves multiple complex sentences that make the argument indeed richer, but also intricate. So, if you want a simple reading, you'll be a little surprised.

The book consists of four parts:
1) Losses
2) Execess
3) Transports
4) The World of the Simple

Each section relates to a different set of neurological problems resulting in a mental disorder.

1) In the "Losses" the author describes nine cases.
"Neurology's favourite word is 'deficit', denoting an impariment or incapacity of neurological function: loss of vision, loss dexterity, loss of identity and myriad other lacks and losses of specific functions (or faculties)."

2) In the "Execess" find five "stories".
"What then of the opposite - an execess or superabundance of functions?"
and next
"... we consider their excesses - not amnesia, but hypermnesia; not agnosia, but hypergnosia; and all the other 'hypers' we can imagine."

3) The chapter "Transports" describes six cases.
"...'transports' - often of poignant intensity, and shot through with personal feeling and meaning - tend to be seen, like drimes, as psychical: as a manifestation, perhaps, of unconscious or preconcious activity..."
and next
"...to be seen as psychoses, or to be brodcast as religious revelations, rather than brought to physicians."

4) And at the end of "The World of the Simple" (four "stories"). Dr. Sacks describes cases of people whose neurological disorders outfitted with extraordinary abilities, such as "seeing the numbers," eidetic memory, an excellent sense of smell, etc. (Ie. brilliant savants).

In his book, Dr. Sacks often refers to the study of well-known Russian psychologist A. R. Luria.

Dr. Sacks style of reasoning, not everyone will like it. Besides, there are many similar books, though written from a different perspective than neurological such as "Tales from the Couch: A Clinical Psychologist's True Stories of Psychopathology" by Dr. Bob Wendorf.

Great gift for potential med/neuroscience students
By Stefanie Hutson
I first read this book in a college Intro to Neuroscience class, and I've been a Dr. Sacks fan ever since. It was sad to hear of his passing recently, but it also reminded me what a great book this is for any potential neuroscience or even more general medical student. I purchased this copy as a gift to a graduating high school student who has expressed an interest in neuroscience. Although most of us will change our minds over the course of college, this is still a great book for getting acquainted with the subject and some of the interesting things that can go wrong with our brains.

The thing I like about this book (and all of his books) is that they manage to do justice to a complex topic while still remaining highly accessible to people of average intelligence and background knowledge. Sacks was also a great writer, with that rare and wonderful ability to know what's interesting about a topic and weave a compelling story. It's a shame he's no longer with us, but I'm sure this book and many of his others will continue to delight curious readers for years to come.

Great stories that tie together really well
By Matthew Beyer
A very well put together collection of stories about unusual changes to the brain... The two points that I really felt were driven home through these stories are:

1. The human brain is not some sort of organ separate from our soul/consciousness/self. It IS our consciousness. When it gets changed, we are changed. I think a lot of people implicitly think of the brain as just something that memorizes dates and facts for school, and don't really think of it as the core of their self.
2. The brain, while impressive, is not really a truthful measurement machine. In reality, it's kind of slapped together from a bunch of co-processors and self delusion to make us think our senses are telling us the true story of the world. If one of those co-processors is damaged, weird stuff happens. For example, the title story. The brain does not observe impartially. Human faces are not processed in the same way that hats are processed. If that part of the brain is damaged, then one can, indeed, mistake their wife for a hat.

One minor nitpick I can think of is that the author spends a lot of time grandly editorializing about the philosophical implications of the stories after telling each one. I would have preferred to keep this a bit more minimal and let the stories speak for themselves - they are full enough without him pointing it out. Another thing that personally kind of puts me off is the fact that the whole book is predicated on the Freud/Jung school of psychology, which I think too often was not really based in observation and scientific method. I am sure you can find all sorts of half-truths and unsupported claims in the book if you really start to dig into and research the stories. Still, it makes for great reading.

And that's the thing; this is chiefly a pop-psychology book. No matter the minor flaws in writing, or patchy science, or flawed observation, it doesn't strongly detract from the central ideas of the book. I would consider this book one of the few universally important books I've read, because the stories themselves cut so close to the core of our life experiences. Not just in the obvious ways (ex. trying to explain to a child why Alzheimer's disease is affecting their grandfather), but also in understanding why we are the way we are.

Sorry, I just realized I made myself guilty of "grandly editorializing about the philosophical implications of the stories"... It's hard not to do!

He brings to life the history of the world organization and [65 years] of America’s hopes for and frustration with the world government… This is a definitive account of the United Nations for a general audience, told by a master.” —Jim Hoagland, chief foreign correspondent, The Washington Post

Decent History of the United Nations
By Andrew
With the Second World War winding down, leaders of the victorious nations met to lay down plans for some kind of world organization. After the deadliest war in world history, people wanted the means to prevent another catastrophe from happening again. Lessons were learned from the past. Even when the United Nations was being created, the absolute failure of its predecessor, the League of Nations, to prevent World War II probably loomed heavily on many peoples' mind.

Meisler's book looks at the major events in the often tumultuous history of the United Nations. The U.N. first came into existence in 1945. It was established with the high hopes of trying to prevent war and seeking peaceful solutions to world conflict. These high hopes were immediately crushed as the Cold War started. Since both the United States and the Soviet Union had veto power in the Security Council, the U.N. was incapable of doing much if it affected either side in the Cold War, which much of the world was.

There were some bright spots. The U.N. was able to claim a moral victory in the Korean War. Although that was blighted slightly by the fact that the U.N. was only able to get involved because the Soviet Union was boycotting Security Council meetings at the time and the U.N. forces essentially became a tool of U.S. foreign policy. The U.N. was also involved in the Suez Crisis and helped bring it to a peaceful end.

Not all events were successes. The U.N. attempted to seek a peace in the Vietnam War, but was pushed aside. Peace only came through direct brokering between American and Vietnamese diplomats without the U.N. The U.N. also got heavily involved in a crisis in the Congo in the 1960s.

The reputation of the United Nations seriously fell during the 1970s and 1980s. With decolonization and independence movements sweeping the world, the U.N. General Assembly became proliferated by Third World governments. Large powers, such as the United States, lost influence. In fact, one observer in the book was quoted as saying the most influential country in the General Assembly during that period was probably Algeria.

After the Cold War ended around 1990, the United Nations was able to flex its muscles a little more in world events. The U.N. was an important player during the Persian Gulf War during 1990-91. The number of peacekeeping missions started to increase. The U.N. was not always as influential as it wanted to be, though. In 2003, the United States would invade Iraq. The invasion was never approved by the Security Council. Then Secretary-General Kofi Annan later called the action "illegal" under the U.N. Charter.

Besides the major events in United Nations history, the book also includes information on the eight men to have served as Secretary-General. The book looks at their basic biographical information, their rise to power and what led to their elections as Secretary-General, and at their style and influence at the U.N. and in world affairs.

I only really have two criticisms of the book. The first is that the book is often written from an American-centric point-of-view. Although it is not really surprising considering the influence the United States has had, a more neutral viewpoint would have been better. The other is that there is little information regarding the history of the U.N.'s more prominent agencies, like UNICEF. Most of the attention is given to the General Assembly and the Security Council.

Overall, I enjoyed this book. It contains some of the most important world events the United Nations has been involved in since its inception and looks at some of the most prominent people to have worked there. I would recommend this book to those interested in the United Nations.

Excellent introductory primer of the international body
By Battleship
Stanley Meisler wrote a informative book with a smooth literary style. This book provides essential information about the formation and history of the international organization. Meisler covers the highlights and lowlights of the administrations of the Secretary Generals. He gives detailed coverage of each leader.

Meisler does an excellent job of identifying the successes and failures of the organization. This gives him great credibility because I've read another book on the organization that glossed over the structural weaknesses and occasional failed military interventions of the organization. The author is never afraid to address controversial topics, such as the connection of Kurt Waldheim with the Nazi Party.

Meisler is a supporter of the organization. He is a realist who celebrates the diplomatic successes of the representative body and gives constructive criticism of how the organization can be more effective and avoid the negative press of the detractors. I found this to be a well-organized with measured and highly credible analysis. I learned a lot about the history of the UN by reading this book. It was very enlightening and it was an easy read.

A Valuable History from Someone Who Watched Up Close the Birth and Growth of the UN
By Claude Forthomme (Nougat)
Stanley Meisler is a professional journalist with experience of the UN since it was created in 1948 and up to the present (this is a revised edition), and none more than him has come to know the United Nations so intimately. He is also gifted with a talent for description and his book never slows down, it is a fascinating read. Highly recommended, particularly if you are interested in the political history of the United Nations. It is however a little short on the other aspect of the UN system, the specialized agencies. There are only passing remarks concerning them. Nonetheless, for anyone interested in a book focused on the UN in New York, this is a must read.

Students learn to listen to sound more carefully. Although almost everybody plays with beautiful tone while tuning the violin, many press the bow and make a terrible sound when they start playing, especially double stops. The first exercises in this book are to practice playing the double stops with beautiful tone while listening to the resonance. There are exercises to shift the fingers between the G string and the E string, moving upward and downward in fifths. There are also many exercises to practice listening to the resonance of open strings.

Purpose of the Book

An example of a digital communications link is an integrated circuit (IC) sending binary-level data to another receiving IC through a microstrip transmission line trace in an FR4 printed circuit board environment. At low data rates that do not challenge the performance edge of communication components, designs are robust. At data rates of 10 Gbit/s (now common in the industry), interrelated design issues in the transmitter, the communication channel, and the receiver become more pronounced.

The engineering design effort in the area of high-speed digital links has been extensive. A field called signal integrity has been identified to help high-speed digital designers understand high-frequency design issues. The ultimate goal is to require fewer design cycles during product development. There are many excellent books in the area of signal integrity for digital communications systems (see the References section in Chapter 1 for a listing of recommended books). This book tackles one important subset of this broad signal integrity field: test and measurement techniques, especially for very high speed systems. It focuses on descriptions of test instrumentation hardware, theory of operation, and applications to digital communications links. The topic of jitter in digital systems is covered extensively.

The primary topics for high-speed physical layer characterization are the following:

  • Bit error ratio measurement
  • High-speed digital waveform analysis
  • Jitter in digital data streams
  • Receiver testing
  • Characterization of the physical interconnection structures

It became clear to the authors of this book that there was not a good single source of reference information on the topic of digital communications test and measurement for the physical layer, and thus this book came into existence. The work combines the collective experience of authors from leading test and measurement organizations (Agilent, Circadiant, and Tektronix), component manufacturers, and university settings. The material in this book has been developed from application notes, seminars, conference presentations, short courses, and unpublished works from the last ten years. Test and measurement equipment companies as well as semiconductor manufacturers and even standards committees are producing a steady stream of application notes on selected high-speed digital test and measurement topics. These notes are often product oriented, and one must draw from a large number of sources to piece together a cohesive coverage of the topic. Much of the material has not had wide circulation to date. A trusted reference has been missing, and this work intends to fill this gap. This book takes the expertise gathered by test and measurement authors that was previously scattered in many places and puts it under a single cover.

This book will be useful for technicians, engineers, and scientists who are involved in the digital communications industry or need to learn about it. The book is designed to address the needs of people new to the field and those intimately familiar with it. Digital communications engineers and technicians spend a good fraction of their lives characterizing their system, subsystem, and component performance. This book serves as a reference that adds cohesion to the wide range of topics that must be understood to succeed in system characterization.

The coverage emphasizes an understanding of how the digital system works, how the test and measurement system is connected, and how an instrument does its job. Understanding instrument architectures and operation gives additional insight on limitations and flexibility of the measurements that can be performed. The book also provides insight into the characteristics of the devices under test. Illustrations are intentionally numerous because the authors believe that visual communication of information is how many people receive information most efficiently.

Organization of Contents

The book is organized around the architecture of a simple digital communications link consisting of a transmitter, a receiver, and a channel. Chapters 1-3 give an introduction that provides background needed to understand later chapters in the book. Chapters 4-8 cover transmitter testing, Chapters 9-12 cover receiver testing, and Chapters 13-15 address characterization of the communication channel and internal computer communications links.

Introduction Chapters

Chapters 1-3 provide a general introduction to digital communications and digital communications systems. These chapters offer a series of definitions, concept descriptions, and specific examples of digital communications link topics. They serve as a foundation for understanding the test and measurement chapters that follow.

Chapter 1 (general introduction): This chapter gives an overview of high-speed digital communications systems. The goal is to set a framework for the more detailed chapters that follow. This chapter also introduces terminology used in the rest of the book.

Chapter 2 (jitter introduction): Jitter is a pressing design problem in high-speed systems. This topic has also seen a large development effort by test and measurement companies in the last five years. Jitter is an extensive subject area and deserves its own introduction.

Chapter 3 (communications link examples): This chapter provides specific discussions on selected serial digital communications links. One often needs to understand some detail of the higher-level system definition and performance even when testing is done on the physical layer.

Transmitter Characterization Chapters

Chapters 4-8 address the characterization of the transmitter portion of a digital communications link.

Chapter 4 (bit error ratio testing): A fundamental property of a digital communications link is the bit error ratio (BER), which is the number of bit errors divided by the total number of bits sent. This chapter along with Appendix A provides a detailed description of the test hardware and test methodology for BER.

Chapter 5 (bit error ratio scanning): This chapter describes techniques to find the system bit error ratio as a function of the digital decision threshold settings in time and voltage. One can obtain information on the signal-to-noise margin that is present in a digital link by scanning the decision thresholds. This can be very time consuming, and considerable discussion is given on performing measurement scans in a reasonable time period.

Chapter 6 (real-time oscilloscopes): This chapter covers high-speed waveform measurements based on real-time oscilloscope architectures. Here very fast analog-to-digital converters (now up to 40 GSamples/s) provide time snapshots of digital waveform segments. Advanced jitter measurement capabilities are described in detail.

Chapter 7 (equivalent-time sampling oscilloscopes): Very high speed waveforms can be reconstructed by a sampling process where data points are taken less frequently compared to real-time oscilloscopes. The sampling architectures and unique instrument capabilities are discussed in detail in this chapter. Finally, applications in digital waveform analysis and jitter component decomposition are given.

Chapter 8 (all-optical sampling oscilloscopes): This chapter departs from the earlier ones in that it concentrates on forward-looking optical sampling techniques that will allow viewing of high-speed optical signals such as 40 Gbit/s data streams with high sensitivity and waveform fidelity. Test and measurement equipment with several hundred gigahertz of bandwidth is now becoming available.

Receiver Characterization Chapters

Chapters 9-12 highlight concepts and measurements for the receiver in a digital communications link.

Chapter 9 (digital clocks, clock recovery, and phase locked loops): This chapter first outlines the characteristics of oscillators used as reference clocks in digital systems. It then analyzes clock recovery circuits using phase locked loops. Finally, this chapter gives the critical measurement parameters used to characterize clock recovery circuits.

Chapter 10 (receiver jitter tolerance characterization): This chapter covers the basic hardware, test methods, and setup for characterizing the jitter tolerance performance for receivers.

Chapter 11 (digital optical receiver sensitivity testing): This chapter develops topics related to the testing of digital optical receivers. It addresses uncertainty values in the bit error ratio measurement and gives data-plotting strategies.

Chapter 12 (stressed receiver testing): Standards groups have specified that receivers be tested with controlled signal degradations. This stressed testing method allows the receiver to be characterized in a more realistic signal environment.

Channel and System Characterization Chapters

Chapters 13 and 14 highlight test techniques for the interconnecting transmission line structures for digital links. Both frequency domain and time domain characterization techniques are given. Chapter 15 covers the specific topic of link characterization for internal computer communication functions.

Chapter 13 (time domain reflectometry TDR and time domain transmission TDT): This chapter covers time domain methods for characterizing the propagation of a signal from the transmitter to the receiver. Step-by-step analysis techniques are given to interpret TDR/TDT displays. Advanced measurement software can also be used to create a discontinuity model for the channel.

Chapter 14 (frequency domain measurements): Frequency domain techniques can be used to obtain the same information as traditional TDRs/TDTs with several other modes of operation. This method is contrasted against the direct time domain measurements discussed in Chapter 13. Several examples of measurement applications are given.

Chapter 15 (communications links inside the PC computer archite...

Excellent, easy to read book on the subject
By Evgeni Stavinov
This is an excellent, easy to read book for a practicing engineer. It covers topics of BER measurement, jitter, principles of high speed oscilloscope operation, and many others, in sufficient detail.
The book has just enough math to understand the subject, but not overloaded with lengthy derivations common in academic papers.

I'm working on transceiver channel characterization at the board level, and so chapters on BER measurements were especially useful.

In the next edition of the book I'd like to see some more discussion and tips on improving transceiver channel characteristics. For example, why several channels coming out of FPGA exhibit very different BER characteristics. Also, more discussion on properties of channel coding (8/10bit, 64/66bit), and how they correlate with PRBS.
